It's simple to get your 5th Grade Passport!
1) Download the Fifth Grade Passport Application and Rules.
2) Submit proof of your 5th grade status (copy of a recent report card OR copy of birth certificate).
3) Submit a $10 processing fee.

IMPORTANT NOTE: Once you recieve the Fifth Grade Passport Booklet in the mail, you must affix a 1"x1" current photo of your fifth grader to the inside cover page, marked "Insert Photo Here." Passport's are not valid without this photo ID.

5th GRADE PASSPORTS WILL BE MAILED TO APPLICANTS STARTING IN MID-NOVEMBER. ALLOW 2-3 WEEKS FOR PROCESSING.
